INSURANCE Sforzo | Dillingham Orthopedics accepts Medicare and most major medical insurance plans. Please contact us to learn more. Some insurance plans require a referral from your primary care physician prior to visiting a specialist. Our staff will assist you in obtaining your referral, if necessary. As a service to you, our office will bill your insurance company. If your insurance has a copayment, the copayment is due at your appointment. We are required by your insurance company to collect these fees as per your terms of your healthcare contract. Additionally, patients are responsible for all balances, including coinsurances and deductibles. For your convenience, we accept cash, personal check, Visa, MasterCard, American Express, Discover, and debit cards. Due to the constant changes in health insurance, it is your responsibility to understand the terms of your health coverage. Please review your coverage terms prior to each visit.
