Scheduling an Appointment

We are available to schedule an appointment from 8am to 4pm. In and effort to make your experience as seamless as possible, please have your insurance information available when calling to schedule an appointment.

What to expect at your first visit

For new patients or patients who have not been to our office in over a year, please plan to arrive 20 minutes prior to your appointment time to complete necessary paperwork. Please bring the following to your appointment:

  • Photo ID
  • Insurance cards
  • List of current medications including any and all vitamins and supplements
  • Any current (less than a year old) diagnostic imaging pertaining to your current condition

Insurance

Sforzo | Dillingham | Stewart Orthopedics + Sports Medicine accepts Medicare and most major medical insurance plans. Please contact us to learn more. Some insurance plans require a referral from your primary care physician prior to visiting a specialist. Our staff will assist you in obtaining your referral, if necessary. As a service to you, our office will bill your insurance company. If your insurance has a copayment, the copayment is due at your appointment. We are required by your insurance company to collect these fees as per your terms of your healthcare contract. Additionally, patients are responsible for all balances, including coinsurances and deductibles. For your convenience, we accept cash, personal check, Visa, MasterCard, American Express, Discover, and debit cards. Due to the constant changes in health insurance, it is your responsibility to understand the terms of your health coverage. Please review your coverage terms prior to each visit. If you should have any questions regarding the coverage of a particular procedure, it is best and most advantageous to call your insurance company for clarification. Their customer service representative will assist you.

Workers’ Compensation

The physicians at Sforzo | Dillingham | Stewart Orthopedics + Sports Medicine offer a designated department to handle Workers’ Compensation cases. Treatment under workers’ compensation requires expert knowledge in medical care and a legal understanding of the intricacies of workers’ compensation in order to provide optimum care and benefits to patients. Our mission is to provide excellent service to workers’ compensation patients and coordinate communication between Nurse Case Managers, Insurance Adjusters, patients, employers, and Sforzo | Dillingham | Stewart Orthopedics + Sports Medicine. We do this by staying focused on the injured employee’s ultimate goal to promptly return to work, using alternate duty recommendations as appropriate without compromising recovery. Keeping the injured worker involved in the process will yield positive results in managing the workers’ compensation claim.
